Jacob M. “Jake” Avalos, 16, of Merry Lake, Camden, died suddenly Monday afternoon at the Community Hospitals and Wellness Center in Montpelier, Ohio. He was born on Jan. 1, 1994, in Toledo, Ohio. He is survived by his parents Jeremy J. and Jennifer J. (Avalos) Anderson of Camden. Jake was attending Camden-Frontier High School having finished his sophomore year. Jake was a letter winner on the varsity football team and participated on the track team. He was a member of Lake View United Brethren Church of Camden.

Jake is survived by his parents, a sister Megan M. Anderson of Camden; Maternal grandparents David and Nancy Avalos and Darlene and Karl Williams both of Camden; also surviving are numerous aunts, uncles and cousins.

He was preceded in death by a paternal grandmother Wilma S. Anderson.

Visitation hours for Jake will be from 2-8 p.m. Thursday at Lake View U.B. Church in Camden. Services will be 1 p.m. Friday at the church with Pastor Travis C. Ambrose to officiate. Interment will be at Riverside Cemetery in Montpelier, Ohio. Preferred donations are to the Camden-Frontier Athletic Boosters. The Thompson Funeral Home in Pioneer is in charge of the arrangements.
