Lieutenant Colonel Flanagan was a member of the 121st Tactical Fighter Squadron. On July 21, 1968, he was the pilot of a North American Super Sabre Fighter (F-100D) on a mission over South Vietnam, when his aircraft crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.

Flanaghan may have been the first Nat'l Guardsman to be killed from MD.
