Mad (as in Madison?) Street, 12th Ward of Chicago, of pneumonia, duration: 2 days, duration of complications: heart disease, place of burial, Rosehill, undertaker F.Busher, dated Chicago, 31st May 1894 by S__ Falls, M.D., 1028 W. Manor St. NOTE: I have estimated his birth based on the above age to be about Dec 1812 to Jan 1813.
