Psychiatrist. A charming, kind doctor who specialized in treating women with depression, he was born in Naples to aristocratic, well-connected parents. A fine skier, tennis player, dancer, art collector, and hard worker, he had by age 20 established himself as an expert on new drug therapy (lithium) and was the assistant diretor of Rome University's psychiatric department.

He met Audrey Hepburn on a cruise of the Greek Islands and they married after a short courtship. He was 10 years her junior but Count Dotti was not viewed as a fortune hunter owing to his own considerable success in the field of medicine and because he was well-off. A few weeks after her divorce from first husband Mel Ferrer, they married in Tolochenaz-sur-Morges, Vaud, Switzerland on January 18, 1969. Their son Luca Dotti was born in February 1970 in Switzerland. She lived in Rome with Andrea until increasing violence in the mid-1970s caused her to move back with her two sons to Switzerland. Dotti continued to practice in Rome despite a kidnapping attempt.

By 1976, his steady stream of affairs with other women took its toll on their marriage and they were divorced in 1980. They maintained contact after the divorce for the sake of their son until she died in 1993. He never remarried and continued to teach at La Sapienza and to treat his patients. He died in Rome of an attack of pancreatitis after undergoing surgery for a perforated intestine.
