Sara was born November 3, 1934 in Sturgeon Bay, a daughter of the late John and Cecilia (Kozak) Maples. She attended St. Joseph's Catholic Grade School, and graduated from Sturgeon Bay High School with the class of 1952. She then furthered her education at Holy Family School of Nursing, earning her nursing degree in 1955. Sara worked as a Registered Nurse at Holy Family Hospital, Park Lawn Nursing Home, and North Ridge Medical & Rehabilitation Center where she retired from in 1997. On May 26, 1956, Sara married Leonard J. Sladky at St. Joseph's Catholic Church in Sturgeon Bay. He preceded her in death on April 21, 2002. Sara was a member of the VFW Auxiliary, Post #6179 of Cato where she served as past president (1972-1973).
Survivors include two sons and a daughter-in-law, Michael Sladky, Lodi, WI, Steven and Sandy Sladky, Green Bay; four grandchildren, Steven Michael, Melissa, Jeremy, and Rebecca; two step granddaughters, Erica and Kristina; and one great grandchild, Lillian Ann Marie Black. She is further survived by one sister, Anita (Ralph) Anschutz; two brothers, Robert Maples, and Tim (Bridget) Maples; several nieces, nephews, great nieces, great nephews, other relatives and friends. In addition to her parents and her husband she was also preceded in death by two children, Mark and Janice; and two sisters-in-law, Esther Maples, and Adeline (Joseph) Shavlik.
Funeral services will be held at 11:00 a.m. on Thursday, Oct. 25, 2012 at St. Anne's Catholic Church, Francis Creek. Officiating at the Mass of Christian Burial will be the Rev. John Becker, with burial to follow in St. Anne's Parish Cemetery, Francis Creek.
