Mrs. Cook is survived by her husband, Maxwell; her mother, Mrs. Louisa Reynolds of Jacksonville; two daughters, Mrs. Ellen Duggins and Mrs. Viola Heinrich, both of Springfield; one son Melvin, of Springfield; two brothers, William Reynolds, of Springfield, and Sam Reynolds, of Indiana; two sisters, Mrs. Lillie Hoyne of Memphis, Tenn., and Mrs. Viola Shrakes, of Springfield, and seven grandchildren.
Remains were taken to the Ellinger and Kunz funeral home, Springfield.
