On 23 Oct 1969 B Company, 158th Aviation Battalion, was tasked with conducting a combat assault with troops from A Company, 2/506th Infantry, into a tight landing zone. On the third lift a UH-1H (tail number 68-16274) landed, discharged its troops, and lifted off. The Huey's skid caught on a stump, causing the aircraft to roll right and crash. While the crewmen survived essentially uninjured, six men from A Company, 2/506th Infantry, were killed by flying fragments of the Huey's main rotor blades:
