Born in Seguin, Texas, one of eight children of William T. and Mary H. Butler. Family moved to Llano in 1884. Two years later James decided to become a doctor, so he began attending Centenary College in Lampasas. Received teaching certificate in 1889, and taught three terms in Llano County. In 1893 began work in a drug store where he studied pharmaceuticals. Studied at State Medical College in Galveston. Came to Bandera County in 1901 and began practicing medicine in Medina. Moved to Tarpley in 1902 to practice there. Delivered 836 Bandera County babies between the years 1903 and 1957.

Married Ethel Hudspeth, daughter of Mr. & Mrs. Street Hudspeth, in 1903. She assisted him in his medical practice.

The Butlers had no children of their own, but adopted the orphaned children of Tom and Annie Dial after the parents died.

Moved to Sinton in 1908, then to Harper in 1909. Graduated from Fort Worth University in 1911. Moved to Bandera the same year. Retired from practicing medicine in 1951 at age 81. Was Bandera County Health Officer for twenty-five years, trustee of Bandera Schools, member of the Methodist Church (served 40 years as steward) and member of the Masonic Lodge for over 58 years.

Served on Bandera County Draft Board as a member and Medical Examiner 1917-1918, at which time he joined the Army in Medical Corps.
