He was the son of Warren T. and Edna Fraze Middleton.
He is survived by his wife Sharon J. Richardson Middleton; three sons; two daughters; two stepsons; one stepdaughter; one brother and one grandchild.
He entered the U.S. Army Air Force in 1942, serving in World War II and the Korean Conflict.
He had been a General Practitioner until 1948, then practiced Pediatrics in Bloomington from 1950 until his death.
He was a 1990 inductee into the Monroe County Hall of Fame on which his plaque reads "Honors (Post-1946) City Council member and physician who was involved with a number of social service projects and organizations including Head Start, Stonebelt Council for Retarded Citizens, Public Health Nurses Association and the Well Baby Clinic."
