Alfred Ellis, aged 19, a bachelor, a Soldier, of 14 Sinfield Avenue, Leeds, died at Jervis Street Hospital, Dublin, on 1 May 1916.
The cause of death was a Bullet through the Brain, Shock, certified.
The informant of his death was a member of the staff of the hospital, who registered it on 11 May 1916.
