John was born near Des Moines, Iowa on October 13, 1875, to James and Adelaide Mellisa (Lewis) Hazlitt. The family which included John, his older brother, Henry, his parents, and grandparents, Harrison and Lucy Lewis, all moved to Kansas about 1880. A sister, Cloey, was born there in 1880 and died at age 4 months. The Hazlitts moved to Emporia, Kansas where another sister, Addie, was born in 1883. In 1888 they moved to Palestine, Texas. James Hazlitt died in Palestine in 1890 and is buried in an overgrown, abandoned cemetery with no headstone found. Adelaide remarried and moved to Florida where she died in 1935 in Largo, Pinellas County, and is believed to be buried in a pauper's grave there.

John's first marriage was to Lillie Pearl Barnhart on July 14, 1900. They had one daughter who died in 1904 at about 3 years of age. They divorced in 1911.

In 1912, John married Mary Ellen Boyes (or Boyce); no known children. I don't know if they divorced or perhaps she died.

His third wife was Sadie Alvina Hill. They had 7 children, Oscar James, Minnie Bell, John Oscar Jr., Hazel Cleo, Reynolds Carlyle, Charles Raymond, and William Henry. Their marriage ended in divorce.

John served in the Spanish American War in CO.1, 1st U.S. Volunteer Infantry.
