The last line loses some in translation but basically says:
Say/Pray a Hail Mary for Him
The inscriptions were translated for me by Fr. Janusz Iwan from St. Hyacinth Parish.
He is from Poland and St. Hyacinth still has a Polish Mass every Sunday @ 9 AM.
