Chana Leiba bas Rabbi Dovid Peretz HaKohen
The top row says Isha Tznuah (a modest woman) Hannah Rabbi Dovid Peretz HaKohan. Her Hebrew name was Hanna Liba (bas means daughter) of Dovid (David) Pereta (usually Hebrew for Philip) Her father was a Kohan (a lineage of Jewish priests all descended from Aaron, the brother of Moses. She she is called a bas kohan (daughter of a priest). He was also a Rabbi. The 4th row says nifter (died) yud dalet (14th) of Elul (Hebrew month of Elul) It gives the Hebrew year, but that is beyond me. If it is important to you I can have someone translate the numbers for me. If appears she came from a religious family. She had a son, Joseph Orlin. He also would be a Kohan. The right to be a Jewish priest is inherited from the father's line.
