I found the death certificate for this memorial. The date of death on the certificate is 26 Apr 1916. The place of death was Lima, Allen, Ohio.

Hello!. I've researched this Kensington Union Cemetery very much (aka West Street Burial Ground, etc etc). It was never known as "Old Brick." If You found a record that also included the name "Old Brick" that means that it is for the Union Harmony Burial Ground, which was part of the Hanover Burial Ground (three cemeteries in one). Old Brick owned Hanover, leased out two sections). You'll need to correct this.
