The Wisconsin State Archives in Madison has the Mendota burial permits from about 1930 to 1960. That's where I got my information. If you are family you should be able to contact the hospital directly and get the information you're looking for. Good luck.

Her DOD was not on her stone so I inquired with the gentleman helping me and he gave me a couple of scenarios. First he said they were not responsible for it, second he said it could have been stolen but he doubted that because the screws for the plate are still in tact, third he said it probably had not been laid. His suggestion was that the monument company probably had not been contacted upon her death. So I would imagine you could call them to have that done. I would think, the funeral home would have the monument companies information. Have a great day!!!!
