The sisters at Saint Francis is Tiffin, OH are very helpful. Basically, his birth name was Michael Boes, then he took the name Brother Anthony when he became a Franciscan monk/friar. I'm not sure about the "1921". He may have died in late December of 1920 and buried in calendar year 1921.
