I have been doing genealogy for over 40 years. When I was a child my parents would take my grandmother to the town she grew up as a child to put flowers on her parents graves. This was always done on "Decoration Day" or Memorial Day. My sister and I would get new summer clothes and it would be freezing!
I suppose that is when it all started. My grandmother had an aunt that was buried all alone. I felt bad that her husband and children lived in a different state. But someone must have came and put flowers on her grave now and then.
