Simon enlisted in Co. D, 17th Massachusetts Infantry, on 22 Jul 1861 at Salisbury, MA. He was described as 5 feet 11 inches tall, with hazel eyes, brown hair and a light complexion. He was promoted to 1st Sergeant and mustered out for disability on 26 Apr 1862 at New Berne, NC.

Simon F. Blake and Mary M. Flanders had a daughter together, Annie Lizzie Blake. Also some records have the name Burnside as a middle name vs. Lizzie.
Annie was born in Danvers, MA on Oct. 28th, 1866.
