Milligan was made famous in the book The Minds of Billy Milligan (1981) by Hugo award winning author Daniel Keyes best known for Flowers for Algernon. Billy is known for pleading insanity to three cases of rape near The Ohio State University campus in Columbus, Ohio in 1977. Claiming to have multiple personality disorder and that his personality Ragen Vadascovinch committed thefts and robberies while Adalana described as a lesbian craving affection was the personality attributed to committing the rapes. Milligan was committed to treatment in 1978 when he was found not guilty by reason of insanity.

After 10 years of treatment in state mental institutions he was released in 1988. Court control was ended in 1991 and Milligan moved to California. Several attempts have been made by Hollywood to adapt Keyes' book in to a movie. In the early 1990s, James Cameron co-wrote a screenplay with Todd Graff for a film version he was to direct, then-titled A Crowded Room (with 'A'). This adaptation never came to be because Cameron was sued by adaptation rights holder Sandy Arcara. Reportedly the movie is back on schedule with Leonardo DiCaprio to play Milligan.

Milligan returned to Columbus and was living in a long term nursing facility at the time of his death from cancer.

Billy's birth father is Johnny Morrison who committed suicide on January 17, 1959.
Billy received the Milligan name from his mother's '4th marriage' to Chalmer J. Milligan . Note: her first and third marriages were to the same man/husband, Dick Jonas .
