John Slusher's date and place of birth is a matter of contest: On Mary Magadline's Delayed Certificate of Birth, which is signed by John and notarized, states his birth as Feb 6, 1868 in Indiana. On his Death Certificate, filled out by his son Rubin Slusher, it is listed as Feb 6, 1869 in Kentucky. No direct confirmation has yet been found for either place or date.

On his death certificate his middle name is listed as 'Melvin' whereas on Mary's birth certificate it is listed as 'Milton'. A listing of his children with Sarah Acord, written by his daughter Joyce, states his name as John Milton Ellet Slusher. Other sources, including Parental records, list his middle name as Madison.

In 1906, John and several of his Aunts, Uncles and siblings filed to be placed on the Miller Guillone Rolls for Native Americans. They were all denied on grounds that their applications were filed after the Rolls had been closed and therefore were not accepted. It was stated in the applications that John's Grandmother, Simeon's mother, Mary "Polly" Jackson was of Native American heritage. No evidence has been found for this that I know of.
