Oliver is survived by parents Louise and Darren Mapp, and older brother Joshua.
Oliver Mapp was on a weekend family visit to wish his great-grandmother happy birthday when the tragedy happened. After he fell from the stairlift his head was trapped between the lift's seat and the wall as the machine continued going up. Oliver's grandfather, who had taken him to the semi-detached home of his great-grandmother Eileeen Meeson, in Telford, Shropshire, desperately tired to revive him. Paramedics were called and the schoolboy was rushed to hospital, but despite further efforts to save him he died. Oliver's parents Louise Mapp, 36, and Darren, 39, were at their home in Bolton Le Sands, Lancashire, at the time. They immediately drove down to Shropshire on hearing of the accident.
