The only child of William Ulick O'Connor Cuffe, 4th Earl of Desart and his first wife, Maria Emma Georgina Preston, on 23 Jul 1895 Kathleen became the wife of Lt. Col. Thomas Edward Milborne-Swinnerton Pilkington (12th Bart.) of Chevet Hall near Wakefield, Yorkshire.

They were the parents of Phyllis, Pamela Irene, Arthur William, and Ulick O'Connor Milborne-Swinnterton-Pilkington.

She was appointed Dame of Grace, Most Venerable Order of the Hospital of St. John of Jerusalem (D. G. St. J.).

Dame Kathleen died at the family's London residence 43 Cheyne Place, Chelsea.
