She was born October 14, 1910 in Tarentum, Pennsylvania, a daughter of Bartolomej and Katarina (Visocky) Kozak. She married John Cebulko on June 12, 1934 at St. Stephen's Church. He died June 11, 1964.
Also preceding her in death were a son: John Jr. in infancy, two brothers and two sisters, besides her parents and husband.
Surviving are three daughters: Joanne (Clarence) Neighbor of Streator, Mary (Gerald) West of Englewood, Colorado, and Carol (John F.) Ramza of Schaumburg; two sons: Edward of Streator and Daniel of Peoria; 15 grandchildren; three brothers: Michael and Andrew of Streator and Albert of Wilmington, Delaware; and a sister: Catherine (John) Leskanich of Streator.
Mrs. Cebulko resided in Streator all of her lifetime and was presently employed by J. Porter Company in Ottawa. She was a member of St. Stephen's church, Polish National Alliance and St. Stephen's Lodge.
