William was the son of John and Anastasia Buckley McCarthy.

William married Mary Frances Barrett 17 May 1908 at Our Lady of the Holy Rosary Church in South Boston, Massachusetts. They had five children: John (b. 1909); William Joseph Jr (b. 1910); Alice (b. 1915); Kathleen (b. 19190); and Paula (b. 1925). The family lived on G Street in South Boston for many years.

William McCarthy was a firefighter for the city of Boston. He was appointed to duty on 3 May 1911, and was a member of Engine Company 10, Beacon Hill. William was seriously injured in the line of duty, fighting a fire on Cambridge Street, Beacon Hill. Two fire fighters were killed in that blaze.

William continued to serve in the BFD until he retired 3 May 1918.
