Declaration for Invalid Army Pension
State of Michigan, County of Eaton, SS:
On this 18th day of March, One Thousand Eight Hundred and Seventy six, personally appeared before me Philo J. Patterson, Clerk of the Circuit Court within and for the County and state aforesaid, Stephen G. Reynolds, aged 38 years, and a resident of Chester MI, County of Eaton, in the State of Michigan who being duly sworn according to law, declares that he is the identical Stephen G. Reynolds, who enlisted in the service of the United States at LaPorte, County of Laporte and State of Indiana, on the 7 day of February, 1865, as a Private in Company "A", commanded by Captain Spain, in the 15 Regiment of Ind. Vols, in the War of 1861, and honorably discharged at Nashville, State of Tenn., on the 19 day of September 1865; that while in the service aforesaid, and in the line of duty as a soldier at or near a place called Tullahoma in the State of Tenn., on June, 1865, Contracted Diarrhea from bad water and irregular diet and exposure, which became chronic, also at same place caught heavy cold, which settled on his lungs and caused lung disease, both of which continue and greatly incapacitate him from performing Manual Labor. Received hospital treatment as follows: In Camp. He has not been in the military or naval, marine or civil service of the United States since the 19 day of September, 1865. Since leaving the service he resided mostly at or near LaPorte Co., Ind. His occupation has been farming. When enrolled he was a farmer. And for the purpose of prosecuting his claim he hereby appoints P. H. Fitzgerald, of Indianapolis, Indiana, his attorney in fact, with full power of substitution, and with authority to receive his Pension Certificate.
