Obituary for Mr. Fortas T. Hazelwood

Fortas Terell Hazelwood was born on March 5,1969, as the youngest of four children born to the union of Janice and Joseph Hazelwood Sr. in Milwaukee, Wisconsin. Fortas received his formal education from Milwaukee Public Schools graduating from Custer High School, Class of 1986. After receiving his diploma, he gained employment with the City of Milwaukee as an Electrician. Fortas worked b several other jobs during his career; he was employed by the United States Postal Service at his passing.


Fortas was blessed with two wonderful children whom he loved dearly, Joshua Hazelwood born September 9, 1990, and Hameeda Hazelwood born August 25, 2002.


Fortas joined the Nation of Islam in the early 90's and remained steadfast in his faith until his passing. Fortas surely belonged to Allah and to HIM he shall return.


Fortas is most fondly remembered for his quick-witted comeback and laid-back nature. In his spare time, Fortas loved watching his home teams, the Bucks, and the Packers play. His family and friends will miss his contagious laugh, listening ear and unconditional love. Fortas Hazelwood was a true soldier, who never complained during his fight with Congestive Heart Failure.


Fortas departed this life on Monday, May 2, 2022, surrounded by his loved ones. He was preceded in death by his parents, Janice and Joseph Hazelwood Sr. and his aunt and uncle Linda and Robert Hall; who were instrumental in raising him; his aunts, Tracy Pate, Nyia Robinson, Odean (Dave) Childs, Verdia (Robert) Bass, Quen Esta Hazelwood, Theresa (Bill) Wigley, Mable Hazelwood; uncles, Leonard

Pate Jr, Nathaniel Hall, Willie(Luella) Hazelwood, Aaron (Betty)Hazelwood Jr. and nephew Jemahl Joseph Hazelwood .


Fortas leaves to cherish his memory his children; Joshua Hazelwood, Hameeda Hazelwood; his grandchildren, Joshua Williams, Karter Wilkins; his siblings, Robin Hazelwood, Portia (Jeffery) Sanders, Joseph Hazelwood Jr (Cyrena), Jeremy Hazelwood, Joalle Hazelwood, Nathaniel Muhammad; his cousins whom he was raised with, Chiquita Smith, Freddie Pate, Jessie Pate, Monica Hall, Robert T. Hall and Edward Pate; his aunts, Mildred Pate, Frankie Hall, Janette Harris, and a host of cousins, nieces, nephews, his childhood friends from Samuel Clemens, Edison and Custer High School.
