Final mission:
On September 21, 1968, a U.S. Marine Corps helicopter CH-46D (tail number 152557) from Marine Medium Helicopter Squadron 262 (HMM-262) was flying a resupply mission in the early afternoon. At 1345 hours after delivering a load of supplies to F Company, 2/3, 10 miles north of Vandegrift Combat Base, the aircraft was on climbout from the landing zone when the pilot, CAPT Donald L. Templeton, reported being hit by enemy fire. He broadcast his intention to fly the aircraft to a secure area if possible. For reasons not precisely determined, the helicopter crashed into a dense jungle mountainside approximately 1000 meters southwest of the landing zone near which he had been hit. All five crewmen, including aircraft commander CAPT Templeton, co-pilot 1LT Stanley G. Lawson, crew chief CPL William M. Berry, gunner CPL Harold E. Feezel, and crewman SGT Leonard F. Skonieki Jr., plus one passenger which had been picked up at F Company's position, PFC Michael R. Page, were killed in the crash.
