Sarah was the daughter of Stephen Caton and Matilda Forrester who were from Cocke County, Tennessee

She was married to Elder Samuel "Sam" McMillan who was from Cocke County, Tennessee. They moved to the North Carolina area in February 1913 according to a notation in the Family history section of the bible of their oldest son Elder Walter Conrad McMillon.

She and her husband had 15 children together. They were in order of birth: Walter "Conard", Stephen B., Levonia, Phoebe, Margret "Matilda", Laura, Cora (or Chora per a family bible), Louthenia, Hester, Belvia, Mary Elizabeth, James, David Columbus "Wade" E., William Thomas "Tom" McMillan and Minnie.

Five of their children are buried at the same cemetery, the Mechanicsville Primitive Baptist Church in High Point, NC.
