The son of Dr. William F. & Maria Laird (Clarkson) Smith, in 1860 he was a doctor living in Lancaster, Lancaster County, Pennsylvania, although the 1863 draft registration lists him as a resident of Chanceford Township, York County, where he was born. He studied medicine in Lancaster and then graduated from the University of Pennsylvania Medical School in 1861.

During the Civil War, he served as assistant surgeon with the 105th Pennsylvania Infantry October 15, 1861, through October 5, 1862, when he tendered his resignation. He also enrolled June 5, 1863, with the 28th Pennsylvania Infantry, and on December 23, 1864, transferred to the 73rd Pennsylvania Infantry at the rank of full surgeon. He honorably discharged July 14, 1865.

He married Hannah M. Murphy October 31, 1865, and set up a medical practice in Airville, York County.
