To Whom It may Concern:

On May 06, 1999 Mrs. Cindy Adams, dedicated employee of American Medical Response of Texas, Houston Operations, was involved in a tragic, fatality accident while she traveled home from work. Cindy, a Texas Department of Health certified Emergency Medical Technician, will be sorely missed by her friends and peers.

Those who knew her will remember Cindy as a caring mother, loyal friend and compassionate provider of patient care. Her charisma and out going personality exemplified her desire to pursue life's journeys to the fullest. In remembrance of our fallen colleague, AMR invites those interested to participate in Cindy's memorial as follows:

Visitation will be Sunday May 09, 1999 from 5:00pm to 7:00pm at
The James Crowder Funeral Home
851 FM 517, Dickinson, Texas 77539.

Services and Processional will be Monday May 10, 1999 at 1:00pm
Burial to follow at Mount Olivet Cemetery, Dickinson, Texas

Those EMS and Fire Department agencies wishing to participate in the processional may contact AMR Houston Communications at (800) 899-2991. Details of apparatus arrangement will be addressed upon arrival at the funeral home by AMR Supervision.
