General Notes: Johnnie Booth (10 Sep 1941 - 18 Sep 1973) served in the US Army from 27 May 1959 to 17 Dec 1965, and again from 18 Apr 1967 to 9 May 1972.

Johnny Booth was the pilot of a helicopter owned by Petroleum Helicopters of Lafayette, Louisiana, that crashed into Timbalier Bay in Louisiana, on 18 September 1973. Both Johnnie and the construction worker he was ferrying to an off-shore oil rig were killed.

Johnnie was survived by his wife Phyllis and son Cary of Dallas, brothers Ray Booth, Dallas, & Phillip Stephen Booth, Sherman, mother Mrs Ethel Booth of Dallas, and father Lloyd R Booth of Ardmore, Oklahoma.
