Private Ballard enlisted on August 1, 1861 at the age of 26 in Montgomery County, NC. He was captured at the Battle of Hanover Court House on May 27, 1862 and sent to Fort Columbus, New York Harbor as a POW. He was subsequently exchanged on August 5, 1862.
Private Ballard was wounded and captured during Pickett's Charge, suffering a gunshot wound to an ankle. On September 20, 1863, he died in captivity of "pyaemia, the result of osteo myelitis" at USA General Hospital, Chester, PA. He was originally buried at Chester Rural Cemetery.
In 1891, he was removed to a mass grave for Confederate POWs at Philadelphia National Cemetery.
