A Makeshift coupling device, that failed to hold was apparently the cause of 55-year-old miner July 28 in Itmann No. 2 mine in Wyoming County.
According the BOM reports, Lemuel Miller, chief electrician at the mine, was attempting to move a mine jitney and portal bus into the mine. He linked the two vehicles together with a chain and track plate as an improvised coupling device. The energized jitney was behind the portal bus.
He was apparently unable to climb an uphill grade because of water on the tracks and energized the portal bus to gain more power. But as he was preceding, the coupling chain became detached.
Miller apparently got off the jitney and attempted to catch the runway portal bus when he was struck and killed by the jitney.
