Zahra stayed in the Eastern Provinces, returning to Rome in 1563 to give a detailed account of his activities and of the late Bishop Buttigieg to Pope Pius IV, who, in appreciation, on November 7, 1564, appointed him Bishop of the then Diocese of Vico Equense, near Naples, which would later be suppressed to the Archdiocese of Sorrento.
He visited Malta in 1573, when he was honoured by the Order of St. John. Zahra died in Naples in 1582.
Fontana, in the 'Sacrum Thestrum Dominicanum' of 1666, lists Zahra as "in Ordine et Orbe vir famosus", while Pirre in his 'Sicilia Sacra' of 1733, calls him "Magister Antonius Melitensis doctrina et moribus perspicuus".
