Born in Rome at the Torre del Campidoglio, the son of renowned astronomer Lorenzo and nephew of Cardinal Pietro, former Vicar General of the Diocese of Rome and Archpriest of Saint John Lateran Basilica, Monsignor Carlo Respighi was ordained priest in 1895. An expert of Gregorian music, he founded together with the Jesuit Angelo De Santi, the review "Rassegna Gregoriana" which he directed until 1914 and collaborated actively in the drafting of the motu proprio "Tra le sollecitudini" of Pope Pius X on sacred music.

Supernumerary of the College of the Pontifical Ceremonial Masters, he was named under-secretary of the Sacred Ceremonial Congregation, a post which he held until 1918, and secretary of the Commission for Sacred Music of the Sacred Congregation of Rites.

Named apostolic protonotary ad instar participantium on May 28, 1917, he succeeded Msgr. Francesco Riggi as Master of Papal Ceremonies the following year, serving until his death.

Rector of the Basilica dei Santi Quattro Coronati al Laterano and canon of the chapter of the Lateran Archbasilica, he passed suddenly away due to a heart attack at Vatican City aged 74, and was laid to rest inside the named Quattro Coronati basilica alongside his uncle, of which he was titular between 1899 and his death in 1913.
