Mr. Ballard was a retired lieutenant colonel from the Kentucky Army National Guard and was a member of the Barbourville First Baptist Church . He was a World War II Army veteran and an avid golfer.
On September 15, 1946, he united in marriage with Mary Lee Dixon in Barbourville and to this union two children were born.
In addition to his parents, he was preceded in death by three brothers, Ray B. Ballard Jr., Jerry Hopper Ballard and Norman Paul Ballard; and two sisters, Ruth Ballard Faulkner and Jane Ballard Mitchell.
Survivors include his wife of 54 years, Mary Lee Ballard of Barbourville; two children, Jack H. Ballard II and Marsha Beth Ballard both of Lexington; one brother, John Marshall Ballard and wife, Faye, of Barbourville; among other loved ones and dear friends who mourn his passing.
Funeral services were conducted in the chapel of the Knox Funeral Home Monday, May 14 at 2 p.m. where the Rev. Bob Lowery officiated. Mr. Ballard was laid to rest in the Barbourville Cemetery with full military honors.
Bearers were members of the First Brigade Pallbearers-Firing Squad from Fort Campbell. Honorary bearers were Donnie Martin, Ray Canady, Larry Hammons, Jim Williams, John Paul Jones, Leon Disney, Homer Vanover, Don Lawson, Sidney K. Back, John Hauser, Ed Boyd, David Ballard, William L. Lusk, E. V. Bargo Jr. and W. W. Elam Jr.
Expressions of sympathy may go to Alzheimer's research in loving memory of Mr. Ballard.
