The youngest son of Rev. George Cook, 1700-1765, first rector of Christ Church, later Middleham Chapel by the time of John's death, and his first wife, Margaret Anderson Woodhall, 1705-1746, both born in Scotland. They immigrated to Maryland in 1743 with their older offspring and it remains undetermined if John was born in Scotland prior to their departure, or in Maryland after their arrival. It also remains possible he was born at sea during the crossing. John was one of six offspring, only his younger sister, Elizabeth Cook Sedwick also having a memorial.
Although much is known of both John's parents and all his siblings, confirmed burial information is lacking, so no other memorials have been created to date.

George married into the prominent Johnson family of Maryland in 1760, his wife Elizabeth "Betsy" Johnson, the sister of future Maryland Governor and Supreme Court Justice, Thomas Johnson III.
His marker, shown left as posted by contributor Cherie Lynn, is inside the chapel itself, so John appears to have been interred beneath the chapel floor by his father, one of several at Middleham.
