There doesn't appear to be any record of a middle name or her maiden name. Not in James Boone's book nor Hazel Straker's book, either. Middle names weren't really a thing in 17th century England.

Sarah Boon "wido" was buried ye 5th of February 1708 and she died aged 80 years, and never had an aching Bone, or decay'd Tooth."
The Boone Society has not been able to find the marriage or birth records for George Boone II and his wife Sarah, and cannot confirm her maiden name of Uppey."

Also known as St. Mary Magdalene, this is the church in which George Boone I and Sarah (Uppey/Opie) Boone baptized three of their children, Henry in 1663, George (II) in 1666, and John in 1673. Henry and John both were buried there; Henry at age four months, and John at age 20. George (I) and Sarah were buried there also; George I in 1696 and Sarah in 1708. No tombstones exist today for this family.
The Boone Society's English Research Committee has copies of the above mentioned baptism and burial records."

Pennsylvania Berks Exeter Monthly Meeting Genealogy of Berks shows that the parents of George Boone III are "of George & Sarah (Uppey). Given that is a secondary source, still NOT CONFIRMED.
