CHARLES KUBACKI
(1908 - 1939)

Charles Kubacki, 31, son of Mr. and Mrs. Frank Kubacki, died Friday afternoon at 2 o'clock at the home of his parents, 805 S. 26th street. He had been in poor health for the last two and one-half years.

He was born in this city Feb. 16, 1908 and for 13 years, prior to his illness, he was employed at the plant of the Aluminum Goods Manufacturing company.

Besides his parents he is survived by two brothers, Joseph, city, and Frank, Chicago, and two sisters, Mrs. Conrad Mazurkiwicz, Milwaukee, and Miss Anton Kubacki, city.

The body was removed to the residence late this afternoon. Funeral services will be held from the home Monday at 8:30 a.m. and at St. Mary's church at 9 o'clock. The Rev. John Landowski will officiate and burial will be in St. Mary's cemetery.

Gentlemen:
In accordance with the duties which are provided for in the Statutes, I beg leave to submit my report of all proceedings and hearings relative to patients committed to the Wisconsin General Hospital for treatment since the filing of my last report up to and including January 1, 1937.
