He is survived by his mother: Debbie (Todd) Miller; one son: Johnathon Collins; two daughters: Eva Ella Ann Collins & Julie Lynn Collins; two brothers: Christopher (Kimberly) Miller & Joel Lewis; one sister: Tricia Lewis; Fiance': Kathleen Smith; grandparents: Ralph & Brenda Miller; one special aunt: Pat Lewis as well as a host of family and friends.
