Mrs. Hazel Anderson Phillips, age 79 of North Wilkesboro, widow of Dual Turner Phillips passed away Wednesday, June 17, 2015 at Woltz Hospice House.

Graveside service will be held 11:30 AM Friday, June 19, 2015 at Yellow Hill Baptist Church with the Rev. Gwyn Anderson, Rev. Eddie Tharpe and Rev. Kenny Anderson officiating. The family will receive friends from 9:00 until 10:00 AM Friday prior to the service at Reins-Sturdivant Funeral Home.

Mrs. Phillips was born June 25, 1935 in Wilkes County to Ruben Blair and Mada Bullis Anderson.

In addition to her parents, she was preceded in death by her husband; Dual, daughter; Donna Joann Phillips, four brothers; Wilburn, Laddie, Ralph and Carl Anderson, and three sisters; Flora Belle Kerley, Floeitha Johnson and Marie Anderson.

She is survived by one sister; Louise Johnson of North Wilkesboro.

In lieu of flowers, memorials may be made to St. Judes Children's Hospital, c/o Window World, 118 Shaver Street, North Wilkesboro, NC 28659 or Wounded Warriers, 4200 Morganton Road, Suite 300, Fayetteville, NC 28314.
