Annie Fain was the only child of Samuel Layfayette and Annie Jackson Fain. With the death of her mother shortly after her birth, Annie was entrusted to the care of her maternal grandmother, Indiana Windrow Jackson, of Rutherford Co., Tn. Her father was a traveling Methodist minister or circuit rider, so it was possbile for him to visit her there. At age 12, Annie went to boarding school at Tennessee Female Institute, in Franklin, Tn. She later attended Ward College (Belmont University) for a time. Then she attended Peabody College (now a part of Vanderbilt) in Nashville. At age 18, she continued her education at Soule College in Murfreesboro, Tn, and was educated in music, art, language, and elocution. Some of her paintings still belong to her family members.

When her father, S.L. Fain, accepted a call to a church in Sparta, Tn., Annie went there and met and married Erasmus Lee Clark on 3 Sept 1896. Their first child, Waman, was born in June of the following year. Her husband, Lee Clark, died shortly before the birth of their second child, a daughter, Sammie Fain. After the death of her husband, Annie Fain Clark returned to the home of her grandmother, Indiana Windrow Jackson, and lived there for about two years.

With the death of her grandmother, Annie Clark took her portion of her grandmother's estate and bought a house in Lawrenceburg, Lawrence Co., Tn., near her father and his second wife, Dante. On 7 Nov 1901, Annie married for a second time to Marshall Simeon McDougal, in Eagleville, Rutherford Co., Tn. Sim McDougal was a widower 22 years her senior. Annie and Mr. Mac had three children:
- Anne Elizabeth McDougal
- Marshall McDougal
- Fain McDougal

In 1914 Mr. McDougal became seriously ill and traveled to Buffalo, New York for treatment. Mr. Mac died there in 1916. Just four months after his death, Annie Clark McDougal fell ill with a brain tumor and died 31 January 1917. She was 42 years old. We read from her obituary, "...Hers was a beautiful character, cultured and refined, with pure and lofty ideas. She was a woman of wide influence and value to the community. Her church, social and civic activities were notable and in all of these relations of life she will be sorely missed. The members of the Mothers Club and Civic League formed an aisle from the church door to the street, through which her body passed from her church for the last time..."
