Dale A. Chizek, age 42, of 1260 S. 32nd St., Manitowoc, died Monday, Aug. 18, 2003, at Holy Family Memorial Medical Center, Manitowoc.
A private family memorial service was held on Friday, Aug. 22, 2003.
Dale was born Aug. 19, 1960, in Manitowoc, son of Liebert and Patricia Terens Chizek. He was a graduate of Valders High School, class of 1978. In 1980, he married the former Pam Dehne. Dale was employed with Oil-Rite Corporation in Manitowoc.
Survivors include his wife: Pam Chizek; six children: Tabitha, Jamie, Adam, Tiffany, Thomas and Ashley Chizek, all of Manitowoc; his parents: Liebert and Patricia Chizek, Manitowoc; his father-in-law and mother-in-law: Lester and Mille Dehne, Newton; one sister and two brothers-in-law: Cindy and Brian Wendt, Manitowoc; Bob Morgan and his wife: Rose, Reedsville; nieces, nephews, other relatives and friends also survive.
The Pfeffer Funeral Home & Cremation Care Center, Manitowoc, is assisting the family with funeral arrangements.
