This record found on ancestry.com has a shipping date of 9-4-1936. It is printed that "this application is for the UNMARKED grave of a veteran. It is understood the stone will be furnished and delivered at the railroad station of steamboat landing above indicated, at Government expense, freight prepaid. I hereby agree to promptly accept the headstone at destination, remove it and properly place same at decedent's grave at my expense. .. " The applicant is Mrs Chas. Brandt, 22643 Barclay st, for 27th Street, Baltimore, Md. on June 24, 1936.
There are ink entries on this application, but then someone went back and added some red ink corrections / markings as well.
