Barbara was the fifth child and second daughter of Dr. John Thornton Gilmer and Lydia Laurie Barker. She was named for her older sister, who died in 1839 at the age of 7y11m, who had in turn been named for their maternal grandmother, Barbara Ann (Walton) Barker.
