Height 5 ft 9.75 inches, 155 lbs, blue eyes fair complexion, dark hair.
Occupation: Hauling Lumber, farmer.
Passed away at the Veterans Administration Facility, Walla Walla, Washington, on 29 June, age 90 years, 11 months, 12 days. Buried at Mountain View Cemetery; their file lists as next of kin - 'grandson' only. No wife listed or buried next to him. Death certificate lists his place of residence as Mason City, Okanogan, Washington.

Phyletus fought in the KS Indian Wars [Cheyenne and Kiowa Campaign, Arapahoes and Comanches] and filed for a pension from Washington State in June of 1917.
