Son of Gov. William Cabell and Agnes Sarah Bell Gamble. He was the grandfather of novelist James Branch Cabell. He attended the College of William & Mary, received his M.D. degree at the University of Virginia in 1833, and worked in Philadelphia and Baltimore before returning home to establish his practice. On 19 Jan 1843, he married Margaret Caskie.
