Known children by wife Caroline Carter Watson include: Dr. John William Watson, Sarah Watson Jacobs, Charlie Milton Watson, Walter Gaston Watson, Martha Frances Watson, Cora Manly Watson Campbell, and Lena Watson.

Known children by wife Sarah Cooper include: Mary Ann Watson Smith, James Eldridge Watson, Joanna Watson McFadden Smith, and Henry Willis Watson.

It is generally accepted that John Watson was the son of Ethelbert "Burt" Watson and Mary Frances Jones Watson, but this has not been formally verified, other than thru family lore. However, the family lore seems to be generally validated by the 1830 US Census of Wake County, North Carolina, because that census shows a Burt Watson with a wife and kids which matches the known family statistics.

Ethelbert was originally believed to be the nephew of Josiah Ogden Watson of Raleigh, North Carolina. However, new research seems to indicate that Josiah and Ethelbert were actually first cousins; Josiah being the nephew of Ethelbert's presumed father. Regardless of their actual kinship, when John's parents died circa 1833, Josiah took John and his siblings under his wing to see after them and their well being.

John W. Watson is the brother of Mary Ann Watson Edwards and Dr. James Gaston Watson.
