Ensign W. D. Sample, USN, was designated Naval Aviator #3098 in 1923. Graduated U.S. Naval Academy, Class of 1919 (graduating in 1918 due to the World War).

Captain Sample commanded the fleet aircraft carriers USS INTREPID (CV-11) and HORNET (CV-12) during World War II.

Commanding Carrier Division 22 with the rank of Rear Admiral, RADM Sample was a passanger on a VPB-205 PBM seaplane which was lost with all hands on the island of Honshu, Japan. The wreckage was not found until 19 November 1948.
