His father was George McCullough, born in PA. His mother was Rebecca Clovis, also born in PA.

Theodore was a farmer. He married Margaret Cleo Meredith on March 19, 1911. According to their marriage license, he was 31 and she was 21 years of age. They were married by Rev. B. F. Hughes at "Forks of road on Forester's Run" in Tyler County.

According to Theodore's death certificate, he died from chronic nephritis and pulmonary fibrosis and a contriubuting secondary cause was influenza. Chronic nephritis is a chronic inflammation of the tissues of the kidney and is frequently associated with a slow, progressive loss of kidney function. He was 54 years old when he passed.
